چکیده

BACKGROUND Thymoma is associated with various paraneoplastic autoimmune disorders. Herein, we report paraneoplastic neuromyelitis optica (NMO) associated with both anti-aquaporin-4 (AQP4) immunoglobulin G (IgG) and type 1 antineuronal nuclear antibody (ANNA-1) in an invasive thymoma patient. CASE PRESENTATION A woman presented with a sudden onset of bilateral progressive visual loss associated with recurrence of invasive thymoma, 6 years after thymectomy and immunosuppressive treatment. AQP4-IgG and ANNA-1 were present in the patient's serum. Visual outcome was poor despite immunosuppressive treatment. Longitudinally extensive transverse myelitis developed 27 months after the onset of visual symptoms. CONCLUSIONS We have reported a case of paraneoplastic NMO associated with both AQP4-IgG and ANNA-1 autoantibodies in an invasive thymoma patient.

Screening for onconeural antibodies in neuromyelitis optica spectrum disorders

BACKGROUND Some so-called "non-classical" paraneoplastic neurological syndromes (PNS), namely optic neuritis and myelitis, clinically overlap with neuromyelitis optica spectrum disorders (NMOSD), and conversely, in cancer-associated NMOSD, a paraneoplastic etiology has been suggested in rare cases. Therefore, we retrospectively investigated the prevalence of onconeural antibodies, which are hig...

Myasthenia gravis-associated neuromyelitis optica-like disease: an immunological link between the central nervous system and muscle?

BACKGROUND Although overt involvement of the central nervous system (CNS) in myasthenia gravis (MG) is considered rare, hyperreflexia is a common and yet unexplained finding. Aquaporin 4 (AQP4), the target autoantigen in neuromyelitis optica, is expressed both in the CNS and in the neuromuscular junction. OBJECTIVES To evaluate the prevalence of even mild CNS involvement in patients with MG a...

Paraneoplastic neuromyelitis optica spectrum disorder associated with stomach carcinoid tumor.

Neuromyelitis optica (NMO), or Devic's syndrome, is an autoimmune central nervous system demyelinating disorder primarily affecting the spinal cord and the optic nerves. It is characterized by the presence of NMO antibodies, alongside clinical and radiological findings. NMO and NMO-spectrum disorders (NMO-SD) have been reported in autoimmune disorders, and are infrequently described as a parane...

Amphiphysin-positive paraneoplastic myelitis and stiff-person syndrome

Finding the underlying etiology of transverse myelitis (TM) can be challenging, as several disorders, including multiple sclerosis (MS), neuromyelitis optica, acute disseminated encephalomyelitis, postvaccine myelitis, Sjögren disease, neurosarcoidosis, infectious myelopathies, or idiopathic TM, may be causative. Paraneoplastic myelopathy, another differential diagnosis, is associated with a br...
